8 Neonatologie

Samenvatting

Neonatologie is het specialisme van de kindergeneeskunde dat zich bezighoudt met de zieke pasgeborenen en hun behandeling. Verbeteringen in de perinatale zorg (de zorg rond de geboorte) hebben ertoe geleid dat de overleving van pasgeboren kinderen sterk is toegenomen.1 Tot ver in de negentiende eeuw stierf 20% van de kinderen voor het eerste levensjaar. In de twintigste eeuw daalde de sterfte sterk door betere sociaaleconomische voorzieningen, kleinere gezinnen, betere hygiëne, vaccinaties en de ontwikkeling van antibiotica. De neonatale sterfte (binnen 28 dagen na de geboorte) bleef lang hoog, vooral ten gevolge van vroeggeboorte en aangeboren afwijkingen. Pas toen technische vooruitgang beademing mogelijk maakte, begon ook de sterfte door vroeggeboorte te dalen. Ontwikkelingen in de kinderchirurgie hebben ervoor gezorgd dat in de laatste decennia van de twintigste eeuw ook de sterfte van kinderen met aangeboren afwijkingen spectaculair is gedaald. Het is te verwachten dat door vooruitgang in de klinische genetica en de prenatale diagnostiek in de komende decennia ook de postnatale sterfte door aangeboren en erfelijke aandoeningen zal dalen. De keerzijde van de daling van de sterfte van de afgelopen periode is dat het aantal kinderen met ontwikkelingsproblemen en chronische ziekten die berusten op schade opgelopen in de perinatale periode, is toegenomen.
