8 Cardiovasculaire revalidatie en de vergrijzing: dienen we onze doelstellingen aan te passen?

Samenvatting

In de geïndustrialiseerde westerse maatschappij begint de vergrijzing stilaan op zowel medisch, sociaal als politiek vlak zijn tol te eisen. Er wordt dan ook steeds meer aandacht geschonken aan deze bevolkingsgroep. Bij een combinatie van een groter aantal ouderen, een lagere mortaliteit en meer ingrepen kan logischerwijze vermoed worden dat steeds meer ouderen aan een ‘geregistreerde’ hartaandoening lijden en dus voor cardiovasculaire revalidatie en verdere secundaire preventie in aanmerking komen. Deze bevolkingsgroep heeft vermoedelijk wel belang bij een specifieke aanpak van de revalidatie. Zij hebben immers meer kans op aanwezigheid van aandoeningen die invloed kunnen hebben op de cardiovasculaire reactie op inspanning en daardoor op hun deelname aan fysieke activiteiten. Door de stijgende kosten ten gevolge van de vergrijzing wordt op maatschappelijk vlak steeds vaker de vraag gesteld hoe deze populatie toch zo lang mogelijk gezond en functioneel kan blijven. Tot op heden worden echter weinig echt oudere patiënten (> 65 jaar) geïncludeerd in cardiovasculaire revalidatieprogramma’s. In een database met 3931 patiënten hebben we de invloed van leeftijd bekeken op het effect van fysieke training bij hartpatiënten uit verschillende leeftijdscategorieen. Er werd voor training een daling vastgesteld van de inspanningscapaciteit met toenemende leeftijd. Dit geldt zowel voor mannen als vrouwen, waarbij de mannen beter scoorden. Ook wat betreft het relatieve effect van fysieke training stelden we de trend vast van slechts een beperkte daling met het toenemen van de leeftijd. Deze trend was dezelfde bij mannen en vrouwen. Bestaande richtlijnen voor fysieke training van oudere personen benadrukken de rol van functie- en krachttraining als aanvulling op de training van het uithoudingsvermogen.
