7 Rompmotoriek na een cerebrovasculair accident

Van klinische ideeën via wetenschappelijk onderzoek naar therapeutische implicaties

Samenvatting

De rompmotoriek blijkt een belangrijke prognostische factor van functioneel herstel na een cerebrovasculair accident (CVA). De rompmotoriek kan na een beroerte klinisch snel, betrouwbaar en valide gemeten worden. Met de Trunk Impairment Scale (TIS), met subschalen statische en dynamische zitbalans en coördinatie, is te beoordelen of een patiënt kan blijven zitten en wordt tevens de selectieve lateroflexie en rotatie van het bovenste en onderste deel van de romp gescoord. De TIS en de subschaal statische zitbalans blijken bij opname in een revalidatiecentrum de belangrijkste voorspeller te zijn van de score op de Barthel-index zes maanden na een CVA, belangrijker dan de score op de Barthel-index bij opname.
Onderzoek met de Trunk Impairment Scale laat zien dat de rompmotoriek na een CVA een herstelpatroon kent dat vergelijkbaar is met dat van de arm- en beenmotoriek en dat van de functionele vaardigheden. Er blijken geen significante verschillen te bestaan tussen het niveau van motorisch en functioneel herstel na een beroerte. Tussen drie en zes maanden na een CVA blijkt er verder geen significante vooruitgang van motorisch en functioneel herstel meer plaats te vinden, maar zelfs een stagnatie en mogelijk zelfs een achteruitgang. Een gerandomiseerd en gecontroleerd pilotonderzoek ten slotte toont aan dat additionele therapie gericht op het verbeteren van de rompmotoriek een significant positief effect heeft op lateroflexie van de romp, uitgevoerd vanuit het bovenste en onderste deel van de romp.
