53 Schouderklachten

Samenvatting

Schouderklachten komen veel voor in de huisartspraktijk. De oorzaak is meestal in het schoudergewricht of de structuren van de schoudergordel zelf te vinden. Minder vaak zijn ze een gevolg van ernstig trauma (fracturen, luxaties), aandoeningen buiten de schouderregio (pijn door longtoptumor of gerefereerde pijn door hartinfarct, longembolie of acute buikproblemen) of systemische reumatische aandoeningen. Precieze pathologisch-anatomische diagnostiek met behulp van een fysisch-diagnostisch onderzoek lukt vaak niet door de grote interdoktervariatie in de uitvoering en interpretatie daarvan en is voor het therapeutisch beleid ook niet noodzakelijk. Met de functionele meer globale diagnostische indeling is in de huisartspraktijk goed te werken.

Globale indeling:

Schouderklachten zonder passieve bewegingsbeperkingen, maar met een pijnlijk abductietraject of pijn aan het einde van de abductie.
Schouderklachten met passieve bewegingsbeperkingen. Zo mogelijk kan nog nader worden gekeken naar:
- voornamelijk beperking van de abductie;
- voornamelijk beperking van de exorotatie;
- combinaties van beide; in dat geval probeert men te beoordelen welke bewegingsbeperking het meest op de voorgrond staat.

Indien bij aanwezigheid van schouderklachten geen functiestoornissen worden gevonden van de glenohumerale structuren, dient rekening te worden gehouden met functiestoornissen van andere delen van de schoudergordel, zoals de cervic(othorac)ale wervels en oorzaken buiten de schouderregio.
