4 Contracturen in de afunctionele arm na een CVA: preventie en behandeling in de revalidatiefase

Samenvatting

Een CVA leidt in ongeveer 60 procent van de gevallen tot een afunctionele arm. Uitblijven van functieherstel maakt de arm vatbaar voor het ontwikkelen van complicaties zoals contracturen. Om complicaties te voorkomen en het herstel van de arm te bevorderen kunnen tijdens de revalidatie verschillende fysiotherapeutische interventies worden toegepast. Helaas zijn er daarvan maar weinig geschikt voor CVA-patiënten die hun arm niet willekeurig kunnen inschakelen. Voor de interventies die wel kunnen worden toegepast (duurrek, repetitieve bilaterale armtraining, elektrostimulatie) is de wetenschappelijke evidentie zwak of gering: niet duidelijk is of deze interventies contractuurvorming voldoende kunnen voorkomen of bestrijden. Voor het toepassen in de fysiotherapeutische praktijk worden in dit hoofdstuk enkele aanbevelingen gedaan, maar duidelijk is dat meer en beter onderzoek noodzakelijk is.
