Samenvatting

Reeds in 1991 beschreven Brennan et al. dat er letsels bij patiënten ontstaan als gevolg van medisch beleid en dat veel letsels het gevolg zijn van zorg die niet aan de standaarden voldoet.^1,2 Deze bevindingen werden later bevestigd door diverse auteurs.^3-7 Wilson et al. pleitten voor het ontwerpen van veiliger gezondheidszorgsystemen, die de patiënt kunnen beschermen tegen (onvermijdelijke) menselijke fouten.⁸ Hoewel het moeilijk is een direct verband aan te tonen tussen varianties of procesafwijkingen en fouten, is het verminderen van variantie door standaardisering volgens het Institute Of Medicine (iom) een goede manier om de kans op medische fouten te verminderen.⁷
