Samenvatting

Pijn op de borst komt vaak voor en lang niet alle mensen gaan ermee naar de dokter. Bij degenen die wel komen, gaat het in meer dan de helft van de gevallen om relatief onschuldige aandoeningen. Van belang voor de huisarts en de SEH-arts is echter om zo snel mogelijk en met een zo groot mogelijke zekerheid cardiale ischemie, longembolie, dissectie van de aorta en pneumothorax uit te sluiten. Alle genoemde aandoeningen vereisen immers snel handelen.
Wat betreft cardiale ischemie is de anamnese het belangrijkste instrument. De negatief voorspellende waarde ervan is, zeker bij een populatie met een lage a-priorikans, zeer groot. De aard van de pijn en bijkomende symptomen, de aanwezigheid van risicofactoren en een cardiale voorgeschiedenis beïnvloeden de kans op een ACS, maar er is geen combinatie van anamnestische verschijnselen en bevindingen bij lichamelijk onderzoek die een acuut coronair syndroom kan aantonen of volledig kan uitsluiten. Een ECG tijdens de pijn is buitengewoon behulpzaam voor het in- en het uitsluiten van een ACS. Sluitend bewijs voor myocardschade heeft men pas in handen na markerbepalingen. Angina pectoris blijft een anamnestische diagnose. Bij diagnostische twijfel staan verschillende diagnostische strategieën ter beschikking.
Een longembolie kan afdoende worden uitgesloten met behulp van een beslisregel en een D-dimeerbepaling, maar om deze aan te tonen is (multidetectie) CT noodzakelijk. Bij twijfel over de aanwezigheid van een pneumothorax kan een X-thorax nuttig zijn, bij verdenking op een dissectie eventueel aangevuld met een D-dimeerbepaling. Bij een wijde aorta op de thoraxfoto zal een CT of MRI moeten volgen. Soms kan de dissectie met echocardiografie worden aangetoond.
Het gaat bij pijn op de borst in eerste instantie om triage. A-priorikansen voor cardiale ischemie zijn leeftijds- en geslachtsspecifiek bekend, de testkarakteristieken van de verschillende onderzoeksmethoden eveneens. Bij grote kans dient zo snel mogelijk te worden gehandeld, bij kleine kans heeft men de tijd om alternatieve verklaringen te onderzoeken.
Indien de klachten recidiveren of persisteren zonder dat er een afwijking wordt gevonden, is gedegen psychiatrische diagnostiek van groot belang.
Bij vermoeden van een reflux en bijbehorende spasmen kan men trachten een therapie ex juvantibus te starten.
