2 Onderzoek en behandeling van patiënten met benigne paroxysmale positionele vertigo

Samenvatting

Benigne paroxysmale positionele vertigo is een frequent voorkomende aandoening van het perifere vestibulaire apparaat. De klachten kunnen in de acute fase zeer uitgesproken zijn en de patiënt ernstig verontrusten, maar kennen over het algemeen een gunstig verloop. Het klachtenpatroon wordt gekenmerkt door bewegingsgebonden vertigo die gepaard gaat met een nystagmus, het optreden van een latentietijd, over het algemeen kortdurende symptomen met een crescendo-decrescendokarakter en uitputbaarheid van de symptomen bij het herhaald uitvoeren van de provocerende bewegingen. Met behulp van een zorgvuldige anamnese en klinisch onderzoek met de positioneringstests en met inachtneming van differentiaaldiagnostiek kan de diagnose gesteld worden.
Het repositioneren van de in de semicirculaire kanalen aanwezige otoconia is de aangewezen behandeling, waarbij het bepalen van de aangedane zijde, het aangedane kanaal en de vorm van de aandoening succesbepalende factoren zijn. Het interpreteren van de uitgelokte nystagmus is hierbij van cruciaal belang. De efficiëntie en veiligheid van de huidige repositionerings- en bevrijdingstechnieken maken dat BPPV op efficiënte wijze behandeld kan worden.
