18 Vermoeidheid bij multiple sclerose: oorzaken, assessment en aanpak

Samenvatting

Vermoeidheid komt dagelijks voor bij meer dan de helft van de mensen met multiple sclerose (MS). In dit hoofdstuk worden de oorsprong, het meten en de behandeling van deze vermoeidheid besproken.
In de literatuur worden diverse verklaringen geopperd voor het ontstaan van vermoeidheid bij MS, waaronder letsels in de hersenen of veranderde reacties van het immuunsysteem. Het lijkt erop dat niet één enkel mechanisme verantwoordelijk is voor de vermoeidheid bij mensen met MS, maar een combinatie van factoren. Bij het meten van de vermoeidheid moet een onderscheid gemaakt worden tussen subjectieve vermoeidheid en de vermoeidheid ten gevolge van fysieke en/of mentale taken. Deze twee categorieën blijken niet sterk met elkaar samen te hangen. Onderzoek heeft aangetoond dat mensen met MS minder actief zijn dan de gezonde populatie. Tevens blijkt dat de subjectieve vermoeidheid van mensen met MS niet volgt op fysieke activiteit, maar hieraan voorafgaat. De resultaten van onderzoek naar het effect van een multidisciplinaire groepsbehandeling voor het omgaan met vermoeidheid wezen erop dat de groepsbehandeling niet méér effect opleverde dan een alternatieve groepsbehandeling waarin niet werd ingegaan op vermoeidheid.
