15 Actieve revalidatie voor chronische lage rugpijn: cognitieve gedragstherapie, fysieke training of beide?

Samenvatting

In dit hoofdstuk wordt een overzicht gegeven van de theoretische achtergronden van de behandeling van beperkingen ten gevolge van chronische lagerugklachten. De tekst komt uit een breder geconcipieerd en gepubliceerd proefschrift en alleen een aantal resultaten en implicaties voor de dagelijkse praktijk worden besproken.
Er bestaat slechts weinig bewijs voor het verlies aan spierkracht en uithoudingsvermogen van de lagerugstrekkers bij mensen met chronische lagerugklachten. Wel wordt bij deze patiënten een verminderde aerobe capaciteit gevonden dan bij de gezonde populatie. Specifieke aerobe training inclusief training van spierkracht en uithoudingsvermogen van de lagerugstrekkers levert echter geen betere behandelresultaten op dan een cognitieve gedragstherapie zonder deze fysieke componenten. Opvallend genoeg blijkt de combinatie van fysieke en cognitieve gedragstherapie geen meerwaarde te hebben boven de afzonderlijke behandelingen. Veel interessanter is dat het behandelresultaat, ook van de fysieke training, voor een belangrijk deel wordt verklaard door een afname van catastroferende gedachten over de pijn. Verder blijken de verwachtingen van de patiënt en geloofwaardigheid van de behandeling voorspellend te zijn voor het behandelresultaat.
