14 Psychosociale aspecten bij chronische pijn: een overzicht

Samenvatting

Pijn heeft een belangrijke alarmfunctie en hierdoor is het mogelijk om efficiënt te reageren op mogelijke fysieke dreiging. Meestal is de pijn van voorbijgaande aard, maar bij een klein aantal patiënten blijft de pijn bestaan en is de kans op volledig herstel klein. Deze patiënten rapporteren klachten op sociaal, affectief en cognitief vlak, en ze zijn verstrikt in een complex van fysieke, emotionele, cognitieve en gedragsproblemen. Het is van groot belang om deze problemen in kaart te brengen vanuit een biopsychosociale denkwijze. Op die manier kan het pijnprobleem vanuit een psychofysiologische (stress), operante (pijngedrag) of cognitieve visie (vrees, aandacht) benaderd worden, en bij elk van deze zienswijzen kunnen relevante behandelingstechnieken naar voren geschoven worden. Verder is het van belang om aandacht te schenken aan de rol van coping (controle versus aanvaarding) en sociaal-communicatieve aspecten (empathie) bij chronische pijn. Implementatie van de bevindingen op die laatste gebieden in de behandeling van chronische pijn staat nog in de kinderschoenen, maar lijkt veelbelovend.
