Samenvatting

Patiënten met chronische ziekten vormen de belangrijkste categorie mensen aan wie artsen zorg geven.
De patiënten geven vooral zelf vorm aan hun ziekte en de gevolgen ervan.
Zelfmanagement heeft gunstige effecten op het beloop van de ziekte.
Het bevorderen van zelfmanagement is dus een waardevol onderdeel van medische zorg aan chronisch zieken.
