13 Nekpijn

Samenvatting

Nekpijn is een veelvoorkomende klacht. De arts ziet hiervan slechts het topje van de ijsberg en hij dient zich telkens af te vragen waarom juist deze patiënt de moeite neemt om hem hiervoor te consulteren. Is het angst voor een ernstige oorzaak, angst voor een chronisch of invaliderend beloop? Als de vraagstelling eenmaal helder is, is het een stuk eenvoudiger bij aspecifieke nekpijn uit te leggen dat verder onderzoek zelden nodig is. Nekpijn betreft meestal aspecifieke nekpijn.

De grote lijn in de diagnostiek van nekpijn is het uitsluiten van specifieke oorzaken. Is dat eenmaal gebeurd op grond van vooral de anamnese, dan volstaat een beperkt lichamelijk onderzoek. Daarbij wordt vooral gekeken naar beperkingen van de mobiliteit. Aanvullende diagnostiek is vaak niet nodig. Verder is het zinvol op basis van bekende prognostische factoren alert te zijn op dreigende chroniciteit en hieraan in het onderzoek apart aandacht te besteden. Het is een uitdaging voor de arts te proberen een chronisch beloop zoveel mogelijk tegen te gaan.
