11 Claudicatio intermittens: de rol van de fysiotherapeut in een netwerk voor gesuperviseerde looptherapie

Samenvatting

Claudicatio intermittens (CI) is een uitingsvorm van perifeer arterieel vaatlijden. Patiënten met CI ervaren pijn of kramp in de kuit, de bil of de bovenbeenspieren tijdens het lopen. De pijn is het gevolg van ischemie in die spieren. Door de klachten kan de patiënt minder ver lopen waardoor het algemene dagelijkse functioneren wordt beperkt en een sociaal isolement dreigt. De meest gegeven behandeling bestaat uit een eenmalig loopadvies (‘sta op en wandel’) door de huisarts of vaatchirurg. Uit onderzoek blijkt echter dat gesuperviseerde looptherapie effectiever is dan een eenmalig loopadvies. Een nadeel is dat de meeste onderzoeken in klinische centra uitgevoerd zijn, wat enkele praktische bezwaren meebrengt. Daarom is in 2003 in de regio Parkstad het Netwerk Looptherapie Parkstad (NLTP) gestart, een multidisciplinair netwerk waarin vaatchirurgen, nurse practitioners en fysiotherapeuten samenwerken. De rol van de fysiotherapeut is meer dan alleen die van oefentherapeut en trainer, hij wordt meer een persoonlijke begeleider en besteedt veel aandacht aan leefstijlveranderingen.
