10 Vermoeidheid bij multipele sclerose

Samenvatting

Vermoeidheid is bij multipele sclerose (MS) een veelvoorkomende klacht en komt in alle fasen van de ziekte voor. Wat de oorzaken zijn van de vermoeidheid is niet bekend. Wel is bekend welke factoren de vermoeidheid kunnen beïnvloeden of in stand houden. Disfunctionele opvattingen over de vermoeidheid en de ziekte, slaapstoornissen en ontregelingen in het slaap-waakritme, te veel of te weinig lichamelijke, mentale of sociale activiteit, pijn en sociale steun kunnen hierbij een rol spelen. Er zijn grote verschillen tussen MS-patiënten met betrekking tot vermoeidheid. Cognitieve gedragstherapie is gericht op verandering van de factoren die de vermoeidheid beïnvloeden. De fysiotherapeut met kennis van zaken kan via activiteitenregulatie ook een rol vervullen in deze behandeling. Gecontroleerd onderzoek zal nog wetenschappelijk bewijs voor de effectiviteit van deze benadering moeten leveren.

In dit hoofdstuk wordt kort besproken wat MS is en wat de rol van vermoeidheid is. Daarna wordt uiteengezet wat uit de literatuur bekend is over de oorzaken en instandhoudende factoren van vermoeidheid bij MS. Vervolgens komt het meten van vermoeidheid aan bod en ten slotte wordt de behandeling door middel van cognitieve gedragstherapie beschreven.
