Patiëntveiligheid wordt in toenemende mate, en terecht, onder de aandacht gebracht. Ondanks het hoge niveau van de gezondheidszorg in de westerse wereld ondervinden te veel patiënten vermijdbare schade tijdens hun ziekenhuisverblijf.
