In dit artikel belichten wij de begeleiding van oncologische patiënten met dyspnoe. Dyspnoe komt veel voor in de palliatieve fase van hartfalen, COPD en kanker. Het aantal patiënten hiermee zal de komende jaren aanzienlijk toenemen. Dyspnoe heeft grote invloed op de kwaliteit van leven en gaat bijna altijd gepaard met angst. Ernstige benauwdheid is niet alleen voor de patiënt en zijn naasten, maar vaak ook voor de hulpverlener een angstige ervaring. Dyspnoe kan veel oorzaken hebben die vaak naast elkaar voorkomen. Kennis van het beloop van de onderliggende aandoening en van pathofysiologische achtergronden is nuttig om de mogelijke oorzaak te kunnen achterhalen. Veel causale behandelingen vinden in overleg met de specialist in het ziekenhuis plaats, afhankelijk van de conditie, prognose en wens van de patiënt. De symptomatische aanpak is gericht op het verlichten van de dyspnoe, op de beleving en de gevolgen hiervan. Morfine speelt hierbij een belangrijke rol. De begeleiding gebeurt in nauwe samenwerking met wijkverpleegkundige en fysiotherapeut.
