Tijdige diagnostiek van dementie in de huisartsenpraktijk

Samenvatting

Scepsis met betrekking tot nut en noodzaak van vroege opsporing van dementie is niet terecht. Niet alleen de patiënt, maar ook en vooral de mantelzorger is zeer gebaat bij een tijdige diagnose. De huisarts hoeft daarvoor niet zijn toevlucht te nemen tot screening of vroegdiagnostiek, maar zou alert moeten zijn bij de eerste verdachte signalen. Naast de drie fasen in het diagnostisch proces – inclusie, differentiaaldiagnose en zorgdiagnostiek – is als vierde fase ook de diagnostiek van de mantelzorger belangrijk.

Wat dat laatste betreft, ontbreekt het huisartsen momenteel vooral aan duidelijke richtlijnen om de zorgsituatie rond een patiënt met dementie op een aanvaardbare manier te benaderen. Zonder deze richtlijnen voelen zij zich niet de meest geschikte hulpverlener om de mantelzorger te ondersteunen, hoewel deze hen wel als zodanig ziet. In elk geval is het belangrijk, de mantelzorger ook apart te consulteren, dus zonder dat de dementerende patiënt erbij aanwezig is.
