Probleemgedrag, wat doe je eraan?

Samenvatting

Probleemgedrag bij dementie kan zowel medicamenteus als niet-medicamenteus behandeld worden. Niet-medicamenteuze behandeling via zogeheten psychosociale interventies verdient de voorkeur. Deze interventies zijn soms direct gericht op de psychische gevolgen van de ziekte, maar kunnen ook worden aangewend om (dag)structuur te bieden of de mantelzorger te ondersteunen (interventie in het systeem).

Voor medicamenteuze behandeling van probleemgedrag zijn verschillende groepen psychofarmaca beschikbaar: antipsychotica, anxiolytica en benzodiazepinen, antidepressiva, anti-epileptica, cholinesteraseremmers en memantine. Met name antipsychotica hebben veel bijwerkingen, zoals sufheid, extrapiramidale bijwerkingen, risico op een cerebrovasculair accident en sterfte, en hun effect op agitatie is beperkt. Voor vrijheidsbeperking als behandeling van probleemgedrag is nauwelijks plaats.
