Verandering in coping tijdens de behandeling van depressie: farmacotherapie of gecombineerde therapie

Abstract

Dit is een onderzoek naar de verandering in coping van depressieve patiënten door farmacotherapie alleen, of door een combinatie van farmacotherapie en psychotherapie. Van beide patintengroepen is niet alleen de ernst van de depressie tijdens de behandeling bepaald, maar zijn ook de verschillende copingstijlen vastgesteld.

Copingstijlen als expressie van emoties, palliatieve reactie en zoeken van sociale steun veranderen tijdens de behandeling niet. Na twintig weken pakken de patiënten van beide groepen de problemen wel actiever aan in vergelijking met het begin van de behandeling. Ook hebben ze meer geruststellende gedachten en vertonen ze een minder passief reactiepatroon. In de gecombineerde conditie vermijden patiënten ook minder. Na twintig weken zijn er verschillen tussen de groepen in passief reactiepatroon, vermijden en actief aanpakken. Voor deze copingstijlen lijkt de gecombineerde behandeling tot betere resultaten te leiden. Nauw hiermee samenhangend is er een duidelijke trend dat de patiënten van de gecombineerde behandeling na twintig weken minder depressieve symptomen hebben en ook in meer gevallen als verbeterd worden beschouwd door de psychiater.

Er is geen samenhang tussen copingpatronen aan het begin van de behandeling en de mate van herstel van depressie. Veranderingen in copingstijlen tijdens de therapie hebben wel een voorspellende waarde voor herstel. Hoe minder passief iemand reageert en hoe meer hij problemen actief aanpakt, des te groter is het herstel.
