21 Tanderosie: voeding en speeksel

Samenvatting

Uit een vervolgstudie onder volwassen patiënten met tanderosie blijkt dat bij het merendeel het erosieproces na zes jaar is voortgeschreden, omdat ze ondanks de voorlichting hun eet- en drinkgewoonten niet hebben aangepast. (Lussi en Schaffner, 2000).
Lokale applicatie van fluoridegels reduceert niet alleen in vitro, maar ook in vivo tanderosie (Jones e.a., 2002). De aangezuurde fluoridegel reduceert de tanderosie meer dan de neutrale gel. Bij patiënten met hyposialie moeten echter bij voorkeur neutrale mondverzorgingsproducten worden toegepast, omdat dit de therapietrouw van de patiënten bevordert (zure producten zijn voor hen vaak pijnlijk) en omdat deze patiënten de remineraliserende eigenschappen van speeksel ontberen. Aanbevolen wordt om een individueel aangepast preventieprogramma op te stellen, waarin adviezen over dieet- en drinkgewoonten, een optimaal fluorideregime, stimulatie van de speekselsecretiesnelheid en het gebruik van bufferende middelen en vooral een persoonlijke motivatie om de tanden niet-destructief te poetsen een plaats hebben (Lussi e.a., 2006).
