Samenvatting

Hart- en vaatziekten zijn in de westerse landen niet alleen bij mannen maar ook bij vrouwen de belangrijkste doodsoorzaak. Atherosclerose begint bij vrouwen gemiddeld tien jaar later dan bij mannen, vooral omdat de endogene oestrogeenspiegels in de vruchtbare levensfase een beschermend effect hebben op de vaatwand. Bij vrouwen treden verreweg de meeste hartinfarcten en CVA’s op na het zestigste jaar. Gemiddeld zijn vrouwen met een hartinfarct ouder dan mannen, hebben zij minder uitgebreide afwijkingen aan de coronaire arteriën en hebben zij een grotere comorbiditeit zoals overgewicht, hypertensie, diabetes mellitus en het metabool syndroom, met als gevolg een hogere mortaliteit bij het acute myocardinfarct en bij coronaire interventies.
