34 Syncope

Samenvatting

Syncope is een symptoom dat wordt gedefinieerd als een voorbijgaand verlies van bewustzijn, dat gewoonlijk aanleiding geeft tot neervallen. Het begin is betrekkelijk snel en ook het herstel is snel, spontaan en volledig. Het algemene onderliggende mechanisme is een voorbijgaande globale cerebrale hypoperfusie. Kenmerkende prodromen zijn duizeligheid, een licht gevoel in het hoofd, wazig zien, onwel voelen, misselijkheid, transpireren, onzeker voelen. Vaak begint de aanval echter zonder prodromen. Omstanders beschrijven de patiënt meestal als bleek. Het verlies van bewustzijn duurt vrijwel altijd kort, ongeveer 10 tot 30 seconden. Myoklonieën tijdens de wegraking treden betrekkelijk vaak op, tot in 90% van de gevallen. Eén op de drie patiënten heeft een amnesie voor de syncope en deze patiënten rapporteren meestal accidenteel vallen, waardoor de diagnose syncope niet gemakkelijk gesteld kan worden.
