4 Hypertensie

Samenvatting

Hypertensie is in de westerse wereld een van de belangrijkste risicofactoren voor cerebrovasculaire aandoeningen en een myocardinfarct. De aanwezigheid van een verhoogde bloeddruk gaat ook gepaard met een grotere kans op het ontwikkelen van hartfalen, nierinsufficiëntie en recidieven van deze aandoeningen. Een meta-analyse van alle beschikbare prospectieve observationele onderzoeken heeft laten zien dat er een lineaire tot zwak loglineaire relatie bestaat tussen de systolische en diastolische bloeddruk en het risico op hart- en vaatziekte en sterfte. Deze relatie geldt voor mannen, vrouwen, ouderen en voor personen met of zonder reeds bekend cardiovasculair lijden. Een langdurige verhoging van de diastolische bloeddruk van 5 mmHg gaat bijvoorbeeld gepaard met een toename van ten minste 34% van het risico op een cerebrovasculair accident (CVA) en met een toename van 21% van het risico op het ontstaan van ischemische hartziekten. Op middelbare leeftijd is er geen drempelwaarde waaronder lagere bloeddrukken niet geassocieerd zijn met een lager risico op CVA of ischemisch hartlijden.
