Kortademigheid bij ouderen

De rol van natriuretische peptiden bij de differentiële diagnostiek

Samenvatting

Kortademigheid bij de oudere patiënt wordt meestal veroorzaakt doorcopd, hartfalen of een combinatie van beide. Het is niet altijd eenvoudig om onderscheid tussen beide aandoeningen te maken en uit onderzoek blijkt dat een verkeerde of gemiste diagnose veel vaker voorkomt dan verwacht. Er is behoefte aan een specifiekere diagnostische test en de vraag is nu welke bijdrage de bepaling van neurohormonen zoalsbnp (B-type natriuretisch peptide) ennt-probnp (N-terminal-probnp) kan leveren. Zo’n bepaling kan hartfalen adequaat uitsluiten en is matig effectief in het aantonen ervan. Met het stijgen van de leeftijd en bij een afgenomen nierfunctie stijgen echter de concentraties neurohormonen in het bloed. Bovendien is er een ‘grijs gebied’ van waarden die te hoog zijn om hartfalen uit te sluiten en te laag om hartfalen aan te tonen. Dat maakt neurohormoonbepalingen soms lastig te interpreteren. Neurohormoonbepalingen zijn dan ook een aanvulling op het huidige diagnostische arsenaal, maar kunnen de gebruikelijke klinische diagnostiek van hartfalen encopd zeker niet vervangen.kwetsbare ouderen verpleeghuis copd hartfalen neurohormonen B-type natriuretisch peptide(bnp)
