6. Invasie van de mandibula door plaveiselcelcarcinoom – onderzoeksmethoden en reconstructie van de mandibula

Samenvatting

Een mondholtecarcinoom kan de mandibula aantasten. Dan dient ook het aangedane stuk bot verwijderd te worden. Een mandibularesectie heeft gevolgen voor de functie en de esthetiek. Verschillende soorten mandibularesecties worden besproken, evenals manieren om de continuïteit van de mandibula te herstellen (o.a. reconstructieplaat, vrije lap). Of, en hoeveel bot is aangedaan, is vaak onduidelijk, omdat mandibula-invasie soms moeilijk zichtbaar te maken is met beeldvorming. De waarde van verschillende soorten preoperatief onderzoek naar botinvasie wordt besproken: die van klinisch onderzoek, conventionele radiografie, CT, MRI, bot SPECT; alsook de voor- en nadelen van deze onderzoeken.
Omdat de verschillende onderzoeken complementaire waarde hebben, werd vervolgens een algoritme gemaakt waarmee mandibula-invasie het best te voorspellen is, met het minste aantal onderzoeken bij patiënten met plaveiselcelcarcinoom tegen of vast aan de mandibula. Dat diagnostische algoritme bleek te starten met CT of MRI, gevolgd door bot SPECT in het geval de eerste scan negatief is.
