Integrated Chronic Care Management bij atriumfibrilleren

Samenvatting

Het aantal patiënten met een chronische aandoening, waaronder ook atriumfibrilleren (AF), is de laatste jaren sterk toegenomen. Chronische zorg gaat vaak gepaard met fragmentatie in de zorg waarbij chronische patiënten de weg kwijtraken in het multidisciplinaire zorgaanbod en de zorg vanuit financieel oogpunt onbeheersbaar lijkt te zijn. Om dit te voorkomen wordt gezocht naar initiatieven om de zorg te reorganiseren. Een van de mogelijkheden is Integrated Chronic Care Management. In het Maastricht UMC+ is een gespecialiseerde polikliniek ontwikkeld voor patiënten met AF, op basis van de principes van integrated care.
