Zusammenfassung
In den meisten Fällen läßt sich frühestens ab dem 3. posttraumatischen Tag erkennen, ob der Patient sich zunehmend stabilisiert und damit eine Phase der ≫Regeneration≪ eintritt, oder ob sich mit zunehmender Instabilität eine ≫Entgleisung≪ anbahnt. Diese ≫kritische≪ oder ≫vulnerable≪ Phase zeigt ein eindeutiges pathophysiologisches Korrelat, welches sich auch in Störungen der humoralen und zellulären Abwehrsysteme widerspiegelt (s. Kap. 10.2.4). Diese Vorgänge können ein fortschreitendes Organversagen nach sich ziehen. Die Lunge gilt hierbei als Schrittmacher auch für die zunehmende Organfunktionsstörung anderer Organe (Leber, Niere, Herz).
