16 Het effect van gedragsmatige oefentherapie bij patiënten met artrose van heup of knie

Samenvatting

In dit hoofdstuk wordt de effectiviteit van gedragsmatige oefentherapie, genaamd GRADIT, beschreven en vergeleken met behandeling volgens de huidige fysiotherapeutische richtlijn, bij patiënten met artrose van heup of knie. De GRADIT-behandeling is gebaseerd op de behandelprincipes van operant leren en deze zijn gecombineerd met oefentherapie en terugkomsessies. Het uiteindelijke doel van GRADIT is het activiteitenniveau van patiënten stapsgewijs verhogen en integreren in het dagelijks leven.

De effectiviteit van GRADIT is onderzocht in een gerandomiseerd klinisch onderzoek waarbij tweehonderd patiënten met artrose van de heup en/of knie betrokken waren. Primaire uitkomstmaten waren pijn, fysiek functioneren en door de patiënt ervaren herstel. Metingen werden verricht in de weken 0, 13, 39 en 65.

Beide behandelingen lieten gunstige effecten zien, zowel op de korte als de lange termijn, en het verschil tussen de groepen was niet significant. Daarom kan geconcludeerd worden dat GRADIT een mogelijke behandeling is voor patiënten met artrose van de heup of knie, met resultaten die vergelijkbaar zijn met de huidige standaardbehandeling.
